Purpose of this notice
This notice describes how protected health information (PHI) about you may be used and disclosed by Oslume Health, and how you can get access to that information.
How we may use and disclose PHI
For treatment: to coordinate care among caregivers, clinicians, and care team members involved in your services.
For payment and operations: to bill for services and to run quality review, training, and administrative functions.
As required by law: including public health, safety, and legal proceedings when legally mandated.
Your rights
You have the right to inspect and request copies of your records, request corrections, request restrictions on certain uses, request confidential communications, receive an accounting of disclosures, and receive a paper copy of this notice.
You may revoke a prior authorization in writing, except where we have already relied on it.
Our duties
We are required by law to maintain the privacy of PHI, provide this notice, follow the terms currently in effect, and notify you following a breach of unsecured PHI.
Complaints
You may file a complaint with our Privacy Officer at privacy@oslumehealth.com or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.
